Purpose Hypnotherapy is committed to providing quality services to you and this policy outlines our ongoing obligations to you in respect of how we manage your Personal Information. This policy applies to employees, contractors, prospective and existing clients.

What is Personal Information and why do we collect it?
Personal Information is information or an opinion that identifies an individual. Examples of Personal Information we collect include names, addresses, email addresses, phone numbers.
This Personal Information is obtained in many ways including interviews, correspondence, by telephone, by email, via our website www.purposehypnotherapy.com.au, from media and publications, and from third parties. We don’t guarantee website links or the policy of authorised third parties.
We collect your Personal Information for the primary purpose of providing our services to you, providing information to our clients and marketing. We may also use your Personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. You may unsubscribe from our mailing/marketing lists at any time by contacting us in writing.
When we collect Personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

There might be certain circumstances where access could be denied, for example if the information released would have a big impact on the privacy of others or the progress of the individual within their treatment plan. If you wish to access your Personal Information, please contact us in writing.

Terms and Conditions
Confidentiality
Purpose Hypnotherapy will make every effort to keep your personal information private. If you wish to have information released, you will be required to sign a consent form before such information will be released. There are some limitations to confidentiality to which you need to be aware of. Your hypnotherapist may consult with a supervisor or other professional hypnotherapist in order to give you the best service. In this event where your hypnotherapist consults another hypnotherapist, no identifying information such as your name will be released.
By accepting this terms, I consent that Purpose Hypnotherpy may release information, as required by law, when the client poses a risk to themselves or others and in case of abuse to children or the elderly, or subpoena or court order is received. The release of information will be limited to only what is necessary by law. I also release Purpose Hypnotherapy from any liability or claims that could be made against them, concerning mental and or physical wellbeing during the treatment that has bee outlined and agree upon now, and in future.
Scope of Practice
I’m aware that Purpose Hypnotherapy is not a medical practitioner of any kind or a psychologist and understand that hypnosis can not ne a replacement for the advise or service of a psychiatrist, psychologist or medical doctor. Purpose Hypnotherapy hold the right to refuse or cease treatment or refer on at any time.
Participation
I give Purpose Hypnotherapy full permission to hypnotise me and use a combination of modalities, knowing that by my full participation in the process and by listening to my personalised recordings, I play a very important role in my overall success of the treatment.
Guarantee
I understand that by attending a minimum of 4 consecutive weekly sessions with Purpose Hypnotherapy there is a high success rate, but Purpose Hypnotherapy can not and does not guarantee results as the individual success depends on different factors that Purpose Hypnotherapy has no control over, including my desire and willingness for changes me.
Audio recordings
I give Purpose Hypnotherapy full permission to audio record the treatment session and the copyright over any copies distributed to me stays with Purpose Hypnotherapy.
